Guidelines for Form 4 Subject Selection Process

Carefully read the following guidelines before proceeding to the online Application Forms.

Dear Parents,

​Before the promotion of your son into Form Four in the new academic year (September 2026), he will have to choose subjects, which he will pursue for his CXC CSEC Level Examinations in May/June 2028. 

While the ideal situation would be to allow each student his choice of subjects, it is often not feasible and on occasion, not in the student’s best interest. As such, students will have to qualify for ALL non-compulsory subject classes.

The Subject Selection process will be done using an Online Application Form.
Use the links below, which corresponds to each of the the respective Form 3 classes.
Please ensure that you click the link for your son's respective class.
The deadline date for the submission of the Online Application Form is Friday 29th May 2026. Changes can be made online on the form up to this date.

Please DO NOT submit multiple forms for the same student. After this date, changes will NOT be allowed.

Additional Information on Scholarship Groups (Form 6)

There are currently 10 National Scholarship Subject Groupings with only 10 scholarships being awarded per Group. If you are considering pursing this option, please review the presentation below, which was done by a former Dean of Form 6, Mr. Balwant.

Information on Business Subjects

In Forms 1 to 3, your sons have not been exposed to any of the Business subjects (Principles of Business, Principles of Accounts, Economics). As such, the St. Mary's College Business Department has created introductory presentations giving some background into what is expected if any of these Business subjects are chosen in Form 4. Please take the time and go through these productions from our very own Form 4 Business Studies Teachers with your sons. Hopefully, this will guide in making the right subject choices for Form 4.

When choosing the five non-compulsory Form 4 subjects, students should consider the following:

Please take note of some common mistakes when completing the online form

Common Mistakes include:​
  • Please do not submit the Application Form more than once. When a Form is submitted, an email confirmation is sent. In this email, there is a link that can be used to make changes if you so desire (check your 'Junk/Spam' folder if you do not see the email).
  • Sometimes, the email address used to complete the form is incorrect. As a result, parents do not receive a copy of their responses, in case they need to make changes.
  • For the First Choice of 5 subjects, some parents enter the same subject twice, so only 4 different subjects were entered.
  • For the Second Choice, some parents enter subjects that were already entered as First Choice subjects. The Second Choice subjects are those that will be offered to your sons who do not get one or more of their First Choice subjects. For example, if a student does not get one of his five subjects from his First Choice, that subject will be replaced by the first subject you chose from the set of Second Choice subjects. Also, if a student does not get 2 of his subjects from his First Choice, these 2 subjects will be replaced by the first 2 subjects entered from his Second Choice subjects, and so on.
